Im serious, but there should be a licensing procedure for procreation. At the vary least doctors should make sure women is detoxed and institutionalised if there was prior addiction to alcohol/drugs
Not everyone should procreate.

My brother died due to complications of Down's Syndrome at age 4 or 5. I was probably inches away from his fate too, stroke of luck to still manage to be 1.87 and be atleast somewhat sentient. Sentient enough to realise i'm subhuman, not sentient enough to do anything about it.

If im being fully honest, originally i wrote my comment as "down syndrome/10", but i thought it was too mean and not fully accurate since most downies look much worse.
